Merakey Total Health accepts Medicaid, Medicare, and Commercial insurances. If you need coverage, we can help you enroll into a plan that is the right fit for you. Merakey Total Health also offers a Sliding Fee Discount program for patients who are unable to pay for care.

What to Expect at Your Appointment?

Please arrive 10-15 minutes early for your appointment so that you have time to check in and verify your information. All patients have will have the opportunity to check in for their appointments online. If you are a new patient, you will be asked to complete additional forms related to your medical history. If you have not completed these forms online prior to your appointment, please arrive 20-30 minutes early.

When you arrive at your appointment, please have the following:

  • A Photo ID 
  • A list of the medications, vitamins, and dietary supplements that you are taking now, or bring your medications with you for review
  • Information about your past or present health issues, hospitalizations, family medical history, and immunizations
  • Health insurance cards
  • Co-payment for services

If you don’t have health insurance, please bring proof of income. This information can be used to help you enroll in medical assistance program or the Merakey Total Health sliding fee scale if available. Examples of proof of income include Federal income tax return; two current pay stubs; unemployment benefit award letter; letter from employer on letterhead; award or benefit letter; or affiliated agency income verification documentation that meets above criteria.

Merakey Total Health may be able to help you apply for medical assistance coverage.
Merakey Total Health offers a sliding fee scale discount program for patients that are unable to afford care. Sliding fee scale discounts for services are based solely on family income and family size. Sliding fee scale discounts are available to patients with all incomes at or below 200% of the federal poverty guidelines. To qualify, patients must share proof of family and gross income information. Examples of proof of income include Federal income tax return; two current pay stubs; unemployment benefit award letter; letter from employer on letterhead; award or benefit letter; or affiliated agency income verification documentation that meets above criteria.

New patients can save time during their first appointment by completing Patient Registration information before your visit. The new Patient Registration package will be sent to you via email or text message to complete online prior to your appointment. If you have not completed the registration package online prior to your appointment, please arrive 20-30 minutes early to complete the forms in the office.
